The 39-year-old "Criminal Minds" star suffered a broken leg in early August when he was struck by a car while riding his bicycle in Los Angeles.
The 911 call came from a witness who said a man was "flat on his face" -- but he wasn't sure if the actor was hit or just fell over.
Moore was rushed to the hospital and treated for his broken leg.
